چکیده

Post-cardiotomy extracorporeal life support (PC-ECLS) in adult patients has been used only rarely but recent data have shown a remarkable increase its use, almost certainly due to improved technology, ease of management, growing familiarity with capability and decreased costs. Trends worldwide in-hospital survival, however, rather than improving, decline some experiences, likely increased use more complex, critically ill suboptimal management. Nevertheless, PC-ECLS is proving be valuable resource for temporary cardiocirculatory respiratory who would otherwise most die. Because comprehensive review might the practitioner, possibly improve patient management this setting, authors attempted create concise, relevant analysis all aspects related PC-ECLS, particular emphasis on indications, technique, avoidance complications, appraisal new approaches ethics, education training. Extracorporeal respiratory support in adult patients

In patients with severe respiratory failure, either hypoxemic or hypercapnic, life support with mechanical ventilation alone can be insufficient to meet their needs, especially if one tries to avoid ventilator settings that can cause injury to the lungs. In those patients, extracorporeal membrane oxygenation (ECMO), which is also very effective in removing carbon dioxide from the blood, can pro...

Extracorporeal life-support in patients requiring CPR.

In today’s Lancet, Yih-Sharng Chen and colleagues show the possible benefi ts of extracorporeal life-support in adults receiving cardiopulmonary resuscitation (CPR) in hospital for more than 10 min for problems of cardiac origin. Irrespective of the advancements made in con ventional CPR, median survivals to discharge after involvement of emergency medical services are only 6·4% for out-of-hosp...

Adult extracorporeal life support: a failed or forgotten concept?

BACKGROUND The extracorporeal membrane oxygenation (ECMO) has been used in the neonatal and childhood periods with excellent results. The adult experience has been modest with inferior immediate results. The intermediate survival, however, has been promising. We have been using the extracorporeal membrane oxygenation for temporary mechanical circulatory support of adults that present with acute...

Extracorporeal life support for patients with acute respiratory distress syndrome: report of a Consensus Conference

The influenza H1N1 epidemics in 2009 led a substantial number of people to develop severe acute respiratory distress syndrome and refractory hypoxemia. In these patients, extracorporeal membrane oxygenation was used as rescue oxygenation therapy. Several randomized clinical trials and observational studies suggested that extracorporeal membrane oxygenation associated with protective mechanical ...

Coagulation Parameter Thresholds Associated with Non-Bleeding in the Eighth Hour of Adult Cardiac Surgical Post-Cardiotomy Extracorporeal Membrane Oxygenation.

Excessive bleeding and allogeneic transfusion during adult post-cardiotomy venoarterial extracorporeal membrane oxygenation (ECMO) are potentially harmful and expensive. Balancing the inhibition of clotting and distinguishing surgical from non-surgical bleeding in post-operative period is difficult. The sensitivity of coagulation tests including Thromboelastography(®) (TEG) to predict chest tub...
